Care Matching

At TouchPointe Home Care Agency (TouchPointe), our Care Matching System assures the best possible match to meet each client's individualized needs.

STEP 1 - Client Information Intake
Our Client Relations Manager collects the basic information about a client's needs over the phone and arranges a free Client Assessment at the client's home or at our office in Rancho Mirage, California.

STEP 2 - Client Assessment
Our Client Relations Manager will conduct an in-depth assessment of the potential client's health condition, medical needs, emergency information, habits, dietary preferences, favorite activities and the type of assistance required to remain independent. We will also meet with the potential client and/or family members to jointly determine the scope of assistance.

During the meeting, we fully explain the referral services available, answer any additional questions and help with important issues.

STEP 3 - Post-Assessment
An Assessment Summary outlines the major aspects of the client's home care needs and his or her lifestyle preferences as well as directions to the client's home.

STEP 4 - The Care Plan
A comprehensive Care Plan is created to reflect specific services, home care schedule, emergency information, meal and other preferences and any special instructions.

STEP 5 - Client Service Agreement
Once the Care Plan has been finalized, the Service and Payment Agreements are reviewed and signed.

STEP 6 - Caregiver Appointment
Based on the Client Assessment, Assessment Summary and Care Plan, our Client Relations Manager selects the caregivers that best suit each client's needs. Clients are welcome to interview potential candidates.

STEP 7 - Caregiver Informational Session
The selected caregiver is provided with the Assessment Summary and Care Plan, and fully briefed on all aspects of the client's needs, preferences and requirements.

STEP 8 - Quality Control
We contact each new client the day after the first day of care, and visit the client's home five to seven days later to evaluate the care and the relationship between caregiver and client. If necessary, the Care Plan is amended to deal with any unforeseen or changing needs of the client.
